  • Purpose of Risk Management Plan

The purpose of this risk management plan is to protect patients from the risks of patient identification errors. Currently, in the overall health-care industry, the failure of correct identification of patients has progressively resulted in medication errors, transfusion errors, testing errors and wrong person attention procedure (Bologna et al., 2001). In light of this, patient identification errors represent a critical health adversity hence posing a threat to patient safety. All patients are vulnerable to these identification errors which are heavily costly in terms of human, economic and social realms.

This risk management plan seeks to generate an overview of the issue and develop criteria of adopting a standard approach and classifications, fundamental tools for obtaining valid and reliable strategies for patient identification errors and subsequent reporting process. Patient identification errors will be categorized into outcomes, their settings of occurrence (inpatient, outpatient), the procedure of the process that is involved and their probability of their occurrence (low, high).

As Battles and Lilford (2003) study noted, Incident reporting and documentation of the near-misses are ideal approaches to mitigating the patient identification errors. In addition, the Healthcare Failure Mode Effect Analysis (HFMEA) and Root Cause Analysis are identified as the most effective strategy for permanently mitigating the risk of patient identification.

  • Scope

The scope of this policy will border on the entire Al Zahra Hospital Dubai healthcare setting practitioners involved in the analysis of patient identification events. The events include intake (registration, Scheduling), clinical encounters (diagnosis, treatment, monitoring, discharge) and post-encounters (referrals, health information exchanges and electronic prescribing.

The risk management program suggested is basically a system approach.  It would facilitate coping up with the medical errors through the potentially critical components strategies including team working, communication skills, evidence-based practice and furthering the strategies for the uncertainties management (Benitez & Malin, 2010). Nevertheless, this effective response will be oriented on a solid and robust risk management policy that focuses on minimization of the different chances available for recurrence of the avoidable medical errors.
